Bile duct injuries are serious complications during cholecystectomy. They can lead to significant morbidity, prolonged hospitalization, and long-term consequences. Early intraoperative recognition is crucial to minimize these risks and improve patient outcomes. Surgeons must be prepared to identify injuries promptly and decide on the most appropriate management strategy. This includes considering options such as primary repair, drainage, or referral to a tertiary center, while recognizing the limits of one’s own experience. Timely decision-making and appropriate intraoperative measures can greatly influence both short- and long-term outcomes for the patient.
