Lesson description:

Technological advances have produced progressively smaller laparoscopic instruments and higher quality imaging, allowing laparoscopic surgeons to perform precise dissection with excellent control of bleeding and of vascular structures.

Adequate hemostasis is essential during laparoscopic procedures to ensure a clear view of the operating field and to optimise patient outcomes. Primary prevention of bleeding through the use of various laparoscopic devices to carefully dissect and identify vascular structures prior to dividing them is fundamental.

In laparoscopy, as in open surgery, several modes of hemostasis are available. Familiarity with each of the various hemostatic procedures, including a basic understanding of the mechanisms behind their use, will ensure proper usage and prevent unforeseen complications.

In this chapter, we will cover laparoscopic dissection and hemostasis techniques with a specific focus on the fundamental principles of electrosurgical devices, practical aspects of commonly used energy devices in various settings, and the integration of energy systems with other devices. The safe use of surgical energy-based devices in the OR is essential to a successful laparoscopic surgeon.
